In this role, you will play a vital part in enhancing patients' quality of life by assessing their needs, developing personalized care plans, and implementing therapeutic interventions across various settings. Whether working in inpatient facilities, outpatient clinics, schools, or home health environments, you will have the chance to make a meaningful difference in the lives of individuals with diverse needs. Responsibilities Conduct comprehensive assessments of patients' physical, cognitive, and sensory functions to identify their unique needs and goals. Develop and implement tailored treatment plans that focus on improving activities of daily living (ADLs), mobility, coordination, and overall independence. Assist patients with ADLs such as dressing, grooming, feeding, and toileting to promote self-sufficiency. Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless transitions across care settings. Work with individuals across the lifespan, including pediatrics (children), geriatrics (older adults), and those with disabilities or developmental conditions such as autism. Provide therapy interventions in various environments including inpatient units, skilled nursing facilities, outpatient clinics, schools, and home health settings. Engage in case management activities by monitoring progress, adjusting care plans as needed, and documenting patient outcomes thoroughly. Support early intervention programs by working closely with children to foster developmental milestones. Apply physiology knowledge and medical terminology accurately to inform treatment strategies aligned with patients' medical conditions. Qualifications Valid licensure or certification as an Occupational Therapist in accordance with state or national requirements. Experience working in acute care settings such as hospitals or ICU units is highly desirable. Prior involvement in post-acute care environments including skilled nursing facilities or home health is preferred. Demonstrated ability to work effectively with individuals with autism, disabilities, or developmental delays. Knowledge of anatomy, physiology, medical terminology, and therapeutic techniques relevant to occupational therapy practice. Proven skills in assisting with ADLs, discharge planning, case management, and patient care documentation. Experience working with diverse populations including pediatrics, geriatrics, and patients requiring outpatient or clinic-based therapy.
