Occupational Therapist - Hospital at Home Program Activa Home Health | Acute-Level Care at Home | Deliver hospital-quality care-from the comfort of home. Activa Home Health is seeking a clinically strong Physical Therapist Occupational Therapist (OT) to support our Hospital at Home Program. In this role, you will deliver hospital-level physical therapy services in patients' homes, with a focus on mobility, balance, fall prevention, and discharge readiness. Provide skilled occupational therapy services for patients receiving hospital-level care in the home
Perform comprehensive OT evaluations focusing on:
ADL and IADL performance
Functional mobility and transfers
Upper extremity strength, coordination, and endurance
Cognitive status, safety awareness, and energy conservation
Develop and implement individualized, goal-driven treatment plans aligned with acute care and discharge readiness
Address functional deficits related to:
Recent hospitalization
Deconditioning
Acute illness or injury
Educate patients and caregivers on:
Adaptive techniques and compensatory strategies
Use of durable medical equipment (DME)
Home safety and fall prevention
Collaborate closely with physicians, nursing, PT, and care management to support safe outcomes and reduce readmissions
Document accurately and timely in compliance with Medicare, hospital-at-home, and agency standards Qualifications Required Active Occupational Therapists (OT) license in the state of Florida Degree from an accredited Occupational Therapy program Current CPR certification Valid driver's license and reliable transportation Strong clinical reasoning, communication, and documentation skills Knowledgeable in providing care to Medicare patients, including familiarity with Medicare regulations, documentation requirements, and patient-centered care standards Preferred / Highly Desired
• Experience in one or more of the following:
• Acute care or hospital-based OT
• Hospital at Home programs
• Home health or transitional care
• Skilled in:
• Functional assessments and discharge planning
•
ADL/IADL
retraining
Cognitive screening and safety assessments
DME recommendations and home modifications
Energy conservation and activity tolerance training
Comfortable managing medically complex patients in a home-based setting
