• Part-time, PRN Overview We are seeking a dedicated and compassionate home health Occupational Therapist to join our healthcare team.

The ideal candidate will possess a strong understanding of anatomy and physiology, with experience in various settings including home health, inpatient care, geriatrics, and skilled nursing facilities. This role is essential in helping patients achieve their rehabilitation goals and improve their quality of life through tailored therapeutic interventions. This is a PRN Position-Flexible schedule! Home health position that requires driving from patient to patient to see them in their home. Duties Assess patients' needs and develop individualized care plans based on their specific conditions and goals. Implement therapeutic interventions to enhance patients' ability to perform daily activities. Collaborate with multidisciplinary teams to ensure comprehensive patient care. Educate patients and their families on treatment plans, exercises, and adaptive techniques. Document patient progress and modify care plans as necessary to achieve optimal outcomes. Provide therapy in various settings including schools, home health environments, ICU, and skilled nursing facilities. Stay current with best practices in occupational therapy and participate in ongoing professional development. Qualifications A degree in Occupational Therapy from an accredited program. Valid state licensure or certification as required for practice. Strong knowledge of anatomy and physiology relevant to occupational therapy. Experience in geriatrics, home health, inpatient settings, or skilled nursing facilities is preferred. Driver's License Familiarity with developing care plans tailored to individual patient needs. Excellent communication skills for effective collaboration with patients and healthcare teams. Ability to work independently as well as part of a team-oriented environment.
