We are seeking a dedicated and compassionate Occupational Therapist to help patients in home health settings regain or improve daily skills following injury, illness, or disability. This role uses the Home Care Home Base EMR and involves a caseload consistent with home health productivity expectations. Minimum Requirements Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program. Active Illinois state OT license required to start the assignment. BLS certification from AHA or ARC may be required. Experience with Home Care Home Base EMR preferred. Experience in home health is preferred but candidates new to the setting will be considered. Ability to meet productivity expectations of approximately 30 visits per week for registered therapists. Ability to travel within the assigned geographic area. Expected mileage varies, approximately 25 miles per day. Orientation attendance required.
Orientation duration:
5 days. Preferred Qualifications Previous home health occupational therapy experience. Familiarity with community-based documentation and care coordination.
