We are seeking experienced field OT's to join our healthcare team as a part-time therapist. McHenry County Hours:

Days - Monday through Friday As a home health care occupational therapist (OT) with Northwest Home Health and Rehab, your visits are tailored to your geographical location and schedule.

Responsibilities include:

Make home visits to patients in designated geographic territories. Help patients develop, improve and maintain the skills needed for daily living. Develop and implement appropriate individualized care plans Continually assess and revise the occupational therapy care plan, and participate with nursing in the multidisciplinary care plan, as appropriate. Educate and instruct patients and family members in rehabilitative care and activities necessary to promote the patient's health, safety and independent living.

Qualifications include:

A current license as a Occupational Therapist in Illinois A minimum of 2 years of recent work experience as an Occupational Therapist or Occupational Therapist Assistant. Home health experience preferred. Ability to work independently and manage time effectively Strong interpersonal skills, organizational skills and problem solving skills

$80.00 - $90.00 per hour
