Responsibilities:
Develop and implement individualized treatment plans to address patients' physical, cognitive, and emotional needs Conduct thorough assessments and evaluations to identify patients' strengths, weaknesses, and goals Design and implement creative and engaging therapy activities to promote patient progress and independence Collaborate with multidisciplinary teams to ensure comprehensive care and support Maintain accurate and timely records of patient progress and treatment plans Participate in patient care conferences and team meetings to share knowledge and best practices
