Assess Functional Abilities:
use standardized assessments, patient interviews, and real-world task simulations to determine the patient's current level of independence and areas of need. +
Develop Treatment Plans:
create personalized treatment plans tailored to the patient's condition, goals, and discharge needs. + Train in Activities of Daily Living (ADLs): guide patients in regaining the ability to perform basic ADLs such as bathing, dressing, grooming, eating, toileting, and transferring (e.g., from bed to chair). +
Recommend Adaptive Equipment:
introduce and train patients to use tools that compensate for functional limitations (i.e., grabbers, reachers , sock aids, dressing sticks, etc.). +
Environmental Modifications:
Suggest changes to the home or hospital room setup to improve safety and accessibility. +
Educate Patients and Families:
train family members or caregivers on how to assist properly without increasing the patient's dependency, and provide guidance for ongoing care at home or in a rehab facility.
