Acclaim Home Health is seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team in Worcester, Massachusetts. This is an excellent opportunity for a skilled clinician who is passionate about helping patients regain independence and improve their quality of life in the comfort of their own homes. As a Home Health Occupational Therapist, you will work one-on-one with patients to evaluate functional needs, develop personalized treatment plans, and provide therapeutic interventions that support daily living and mobility. Responsibilities Evaluate patients' functional status and occupational therapy needs in the home setting Develop individualized treatment plans and goals Provide therapeutic exercises and adaptive training Educate patients and caregivers on safety, mobility, and activities of daily living Recommend adaptive equipment and home modifications Collaborate with physicians, nurses, physical therapists, speech therapists, and caregivers Complete accurate and timely clinical documentation Monitor patient progress and adjust care plans as needed Qualifications Current Massachusetts Occupational Therapist (OT) license Graduate of an accredited Occupational Therapy program Home health experience preferred, but not required Strong communication and interpersonal skills Valid driver's license and reliable transportation Ability to work independently and manage time effectively Why Join Acclaim Home Health?
