36 Hours Guaranteed (on a 40-hour work week)Pay Rate:
$48•49/hrMileage Reimbursement:
Mileage is reimbursed for travel between visits, from the first patient of the day to the last patient. Job Overview We are seeking a dedicated and qualified Occupational Therapist to join the team. In this role, you will be responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living (ADLs), fine motor coordination, home assessments, and adaptive equipment. You will deliver high-quality, patient-centered occupational therapy services directly in the home setting, working collaboratively with the interdisciplinary healthcare team. Key Responsibilities •Evaluations & Assessments:
Conduct patient assessments to evaluate the level of function by applying diagnostic and prognostic functional ability tests. Evaluate the status of all body systems as required for CMS documentation. •Care Plan Development:
Consult with the physician in the development of the therapy plan of care to address functional limitations and occupational therapy needs. •Skilled Interventions:
Provide services within the scope of practice defined by state laws and in accordance with the established plan of care, utilizing evidence-based techniques. •Monitoring & Reporting:
Observe, record, and report the patient's response to treatment and any changes in the patient's condition to the nurse supervisor and the physician. •Home Assessments:
Evaluate the home environment for safety risks, need for adaptive equipment, and support for daily living activities. •Care Coordination:
Collaborate with other members of the healthcare team to ensure a seamless and patient-centered continuum of care. Required Qualifications •Licensure:
Current and active Occupational Therapist (OT) license. •Experience:
Minimum of 1 year of recent home health care experience. •Technical Skills:
