Provides patient/caregiver education and arranges for adaptive equipment as needed. Qualifications Required Bachelor's Degree or equivalent from an accredited occupational therapy program. LIC-Occupational Therapist

• STATE_MI State of Michigan Upon Hire required CRT-Occupational Therapist, Registered (OTR)
• NB-COT National Board for Certification in Occupational Therapy Upon Hire required LIC-Driver's License
• STATE_MI State of Michigan Home Care Business Lines Upon Hire required Basic Life Support (BLS) within 90 days of hire required CRT-Hand Therapist, Certified (CHT)
