Occupational Homecare Therapist University Home Care Inc - 2.0 Livonia, MI Job Details Full-time $80 - $100 an hour 14 hours ago Qualifications Driver's License OTR Occupational Therapy License Full Job Description Position Summary The Home Care Occupational Therapist provides skilled occupational therapy services to patients in their homes. The therapist evaluates functional abilities, identifies barriers to independence, and implements individualized treatment plans to improve safety and participation in daily living activities. Essential Duties Conduct occupational therapy evaluations in the home setting Develop individualized treatment plans based on patient needs and goals Provide interventions to improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) Assess home environments for safety risks and recommend modifications Train patients and caregivers in adaptive techniques, compensatory strategies, and safe mobility Recommend and instruct on use of adaptive equipment and assistive devices Document evaluations, progress notes, and discharge summaries accurately and timely Collaborate with physicians, nurses, physical therapists, and other care team members Reassess and adjust treatment plans based on patient progress Educate patients and families on fall prevention and functional maintenance Follow all agency policies, state regulations, and reimbursement requirements Qualifications Degree in Occupational Therapy from an accredited program (BS, MS, or OTD) Current Occupational Therapist license in the state of practice NBCOT certification (as required by state) Valid driver's license and reliable transportation Ability to travel to patient homes within assigned territory Preferred Experience Experience in home health or rehabilitation settings Experience with geriatric, neurological, or orthopedic populations Familiarity with electronic medical record (EMR) systems Ability to work independently in a field-based role Strong clinical reasoning, communication, and time management skills Work Environment Field-based position with daily travel to patient homes Flexible scheduling based on patient caseload Varied home environments requiring adaptability Physical activity including standing, bending, and assisting patients during treatment
