We are seeking a professional who provides skilled therapy services to patients in their residences, on an intermittent basis. The Therapist evaluates, plans and implements care to rehabilitate the patient, seeking to promote optimal level of mobility. Confers with physicians and other members of the health car team to formulate realistic goals and evaluate therapy program. The ideal candidate will have Home Care experience and the following qualifications: Willing to travel to multiple locations ( Milford, South Lyon, Wixom and surrounding areas).
