Field Based Compensation (Productivity Points): $75.00 / point | $100,000-$130,000/year We're fixing home healthcare, and we're building our presence in Detroit Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We're building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. The mission of this role Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. Objectives Carry a full, high-quality caseload: ramp to ~28 productivity points/week at target visit quality. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
Be a referral-source ambassador:
every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
Set the standard others copy:
your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. What you'll own Complete in-home OT evaluations and deliver skilled OT visits. Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence. Recommend and train on adaptive equipment and home modifications as needed. Coordinate with nursing and the broader care team when condition or needs change. Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent. Who you are / how you operate Dedicated clinician: home health experience strongly preferred High ownership: you treat your caseload like your own panel and your pod like your own business.

Must-haves Active Michigan OT license (in good standing) Strong clinical judgment and comfort working independently in the field Comfort escalating changes in condition to RN/MD and documenting rationale clearly Strong communication and documentation Reliable follow-through on orders, frequencies, and patient goals Valid driver's license, auto insurance, and reliable transportation and comfort with daily travel across assigned territory. Able to carry/transport basic therapy equipment as needed Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education) Nice-to-haves Prior home health experience Homecare Homebase (HCHB) experience (or similar home health EMR) Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training) Strong organization and time management Our guarantee Full-time target: ~28 productivity points/week ; clinicians earn more for exceeding it.
Guaranteed floor:
~24 points/week for the first 90 days - if your caseload hasn't filled, you're still paid the floor. The rest of the package $1k sign-on bonus paid after first 90 days $1k bonus for referrals (we're growing)! Benefits for full-time (medical/dental/vision, PTO)
Compensation Range:
$100K - $130K
