Build meaningful one-on-one relationships with patients and caregivers. Greater autonomy and clinical decision-making. Join an experienced interdisciplinary team committed to exceptional outcomes. What You'll Do Complete occupational therapy evaluations and treatment visits in patient homes. Address ADLs, home safety, adaptive equipment needs, fall prevention, and functional independence. Develop individualized treatment plans that support patient goals and recovery. Educate patients and caregivers on techniques that improve safety and quality of life. Collaborate with nurses, therapists, physicians, and other healthcare professionals. Complete timely documentation and contribute to positive patient outcomes. What We're Looking For Current Minnesota Occupational Therapist (OTR/L) license. Strong assessment, treatment planning, and patient education skills. Home health experience welcomed but not required. Comfortable working independently and managing a visit-based schedule. Reliable transportation, valid driver's license, and current auto insurance. Passion for helping patients remain safe, independent, and successful at home.
