St. Clare Living Community of Mora is a non-profit provider of skilled nursing care and therapy services. We serve short stay, long term care, assisted living and home health mature clients experiencing medical challenges. We offer comprehensive medical and rehabilitative services for both the long term resident looking to make this their home, and short stay residents who need additional services after a recent hospital stay. St. Clare Living Community is a values driven organization offering healthcare jobs in Mora, MN and its surrounding areas. Duties Conduct comprehensive evaluations of patients' physical, cognitive, and sensory abilities to develop tailored occupational therapy plans. Assist patients with activities of daily living (ADLs), including dressing, grooming, feeding, and mobility to promote independence. Design and implement therapeutic interventions for our populations recovering from injury, surgery or illness. Collaborate with multidisciplinary teams to coordinate care plans, discharge planning, and case management strategies ensuring seamless patient transitions across settings. Provide education and training to patients and families on adaptive techniques, equipment use, and safety measures at home or in clinical environments. Document patient progress meticulously in compliance with healthcare regulations and maintain accurate medical records.
Pay:
$37.00 - $43.00 per hour
