What You'll Be DoingHelp Patients Regain Independence Perform comprehensive in-home occupational therapy evaluations Develop individualized treatment plans focused on safety and ADLs Deliver hands-on interventions to improve coordination, motor skills, and functional abilities Conduct home safety assessments and recommend adaptive equipment or modifications Help patients maximize their independence and quality of life Collaborate With An Exceptional Team Work closely with Physical Therapists, nurses, physicians, and interdisciplinary clinicians Educate patients and caregivers on compensatory techniques and safety Document care accurately using EMR systems Contribute to exceptional patient outcomes and satisfaction Required Qualifications Active Minnesota Occupational Therapist license Valid driver's license and reliable transportation Strong clinical reasoning and communication skills Ability to work independently with confidence Preferred Qualifications Home health experience EMR experience Experience with geriatric or medically complex patients Passion for relationship-based, patient-centered care Occupational Therapy Changes More Than Function.
