The ideal candidate will possess a strong foundation in patient-centered care, with expertise in assessing and improving functional abilities across diverse populations. As an Occupational Therapist, you will play a vital role in developing individualized treatment plans, facilitating recovery, and promoting independence for patients of all ages. This position offers an opportunity to work in various settings, including inpatient facilities, outpatient clinics, schools, and home health environments, ensuring comprehensive care delivery. Responsibilities Conduct thorough assessments of patients' physical, cognitive, and emotional capabilities to identify their unique needs. Develop and implement personalized care plans aimed at enhancing daily living activities (ADLs), motor skills, and functional independence. Assist patients with activities such as dressing, grooming, mobility, and other essential daily tasks to support their recovery or adaptation process. Collaborate with interdisciplinary teams—including speech therapists, physiotherapists, and medical staff—to coordinate comprehensive treatment strategies. Facilitate discharge planning by preparing patients for transition back to home or community settings while ensuring continuity of care. Work with individuals with disabilities or developmental conditions such as autism to improve functional skills and promote participation in daily life. Provide education and training to patients, families, and caregivers on therapeutic exercises and adaptive techniques. Maintain accurate documentation of patient progress, treatment outcomes, and care plans in compliance with healthcare regulations. Experience Proven experience working as an Occupational Therapist across various clinical environments such as inpatient units, outpatient clinics, skilled nursing facilities, or home health settings. Demonstrated expertise in assisting with ADLs and discharge planning for diverse populations including geriatrics, pediatrics, and individuals with disabilities. Experience working with individuals with autism or developmental delays within early intervention or school-based programs. Knowledge of physiology, anatomy, and medical terminology relevant to occupational therapy practices. Familiarity with acute care settings such as ICU or medical-surgical units is highly desirable. Prior involvement in post-acute care or outpatient therapy services focusing on patient care optimization. Skilled in case management approaches that support patient recovery trajectories across different healthcare environments.
