Job Description:
We are seeking a dedicated Occupational Therapist (OT) to provide skilled rehabilitative services in a Long Term Care setting. The successful candidate will evaluate and treat patients with cognitive and activities of daily living (ADL) physical functioning disorders, developing personalized care plans adhering to regulatory and clinical standards.
Responsibilities:
Screen, evaluate, and develop treatment plans for patients with cognitive and ADL physical functioning disorders. Utilize standardized assessments and evidence-based practices to guide clinical interventions. Provide medically necessary, rehabilitative treatments including ADL retraining, therapeutic activities, exercises, modalities, and the use of supportive or assistive devices. Monitor patient progress, adjust treatments, and document outcomes in accordance with regulatory and clinical guidelines. Conduct patient and family consultations, offering counseling on physical health issues. Communicate effectively with supervisors and healthcare team members regarding patient status and care plans. Prepare comprehensive discharge summaries documenting services rendered, following all clinical and regulatory requirements. Maintain accurate documentation of all treatments and services within electronic health records. Record treatment minutes precisely, in accordance with applicable regulations. Participate in clinical meetings such as care conferences and utilization reviews; contribute to facility and corporate training sessions. Obtain necessary durable medical equipment (DME) and supplies as required. Supervise and support Occupational Therapist Assistants, rehabilitation aides, and students as applicable. Comply with NBCOT and state practice acts, professional ethics, and conduct standards.
