Occupational Therapist (OT) - Mixed Caseload (Hands, Outpatient, SNF)

Job Summary We are seeking a licensed Occupational Therapist to join our interdisciplinary rehabilitation team in a permanent, full-time position. This role offers a diverse mixed caseload , including hand therapy, outpatient adult rehabilitation, and inpatient skilled nursing facility (SNF) services . The ideal candidate is flexible, patient-centered, and motivated to provide high-quality, evidence-based care across multiple settings. Responsibilities Evaluate and treat adult patients with a variety of orthopedic, neurological, and functional conditions Provide hand therapy services, including post-surgical and non-surgical upper extremity conditions (experience preferred, training available if needed) Deliver outpatient adult rehabilitation focused on functional independence and return to daily activities Provide inpatient SNF therapy services, collaborating with nursing and interdisciplinary teams Develop and implement individualized treatment plans Document evaluations, progress notes, and discharge summaries in a timely manner Educate patients and caregivers on home exercise programs and adaptive strategies Maintain compliance with state licensure, facility policies, and payer requirements Qualifications Graduate of an accredited Occupational Therapy program Current state Occupational Therapist license (or eligible) Experience or interest in hand therapy, outpatient rehab, and SNF settings Strong communication, documentation, and time-management skills Ability to work independently and as part of a collaborative team New graduates encouraged to apply!
