What you will do:
- Conduct assessments and evaluations in clients' homes to identify their needs for housing adaptations and equipment.
- Design and implement bespoke adaptation plans that enable clients to live independently and safely.
- Collaborate closely with social care teams contractors and suppliers to ensure timely and effective delivery of equipment.
- Provide expert guidance on moving and handling techniques ensuring that all safety protocols are adhered to.
- Offer training and support to clients and their families on how to use adapted equipment effectively.
- Monitor and review the effectiveness of the adaptations and equipment making necessary adjustments to improve functionality.
