Weekend, Minimum of 2 days per month Innovate Physical Therapy is looking for a compassionate and motivated Occupational Therapist to join our team in Bellevue, NE. Formerly known as Hillcrest Physical Therapy, Innovate Physical Therapy is a locally owned outpatient therapy provider offering physical, occupational, and speech therapy services. Our experienced clinicians provide individualized, evidence-based care in a modern Bellevue clinic and through Innovate @ Home, supporting clients in private residences, independent living, and assisted living settings. We are looking for an Occupational Therapist who values clinical excellence, teamwork, and meaningful client relationships. What You'll Do As an Occupational Therapist, you will assess clients and create individualized treatment plans that help them build or regain the skills needed for daily living, mobility, self-care, and independence. You will use evidence-based treatment approaches while collaborating with an interdisciplinary team to support strong outcomes and successful transitions of care.
You will:
Complete occupational therapy evaluations using appropriate tests and measures to establish timely plans of care. Provide treatment interventions that support activities of daily living, functional mobility, homemaking, vocational skills, sensorimotor development, and adjustment to disability. Progress treatment plans using evidence-based guidelines, reimbursement requirements, and client goals. Educate clients and family members using effective, culturally sensitive communication that considers individual cognitive needs. Complete accurate documentation and charging to support therapy services. Collaborate with physicians, therapy team members, and other interdisciplinary partners to support each client's care plan. Plan safe and effective transitions within or outside Hillcrest service lines, including recommended services, equipment needs, and handoff communication. Supervise, collaborate with, and delegate appropriate tasks to therapy assistants, technicians, and support personnel in accordance with regulations. Identify clients who may benefit from additional medical or therapy services and complete screenings as needed. Manage an efficient client schedule while considering safety, preferences, environmental needs, and productivity expectations. Identify risks related to client care, infection control, equipment, documentation, and safety, then take appropriate action. Participate in professional development, clinical education, advocacy, presentations, or other activities that support the therapy profession. What We're Looking For We are looking for an Occupational Therapist who is clinically curious, compassionate, and committed to helping aging adults live as independently as possible. Ideal candidates will have:
A passion for helping residents regain skills, confidence, and independence. Strong communication and relationship-building skills with residents, families, providers, and team members. A willingness to learn, ask questions, and grow within a collaborative therapy environment. Strong clinical judgment or the desire to build it through mentorship and experience. Ability to stay organized, complete accurate documentation, and manage competing priorities. A calm, flexible, and professional approach in changing or stressful situations. Previous experience in skilled nursing, post-acute care, hospital, home health, outpatient, or a similar setting is welcomed but not required. Education & Credentials Graduate of an accredited Occupational Therapy program required. Active Occupational Therapist license in Nebraska or a compact state required. Basic Life Support certification through the American Heart Association required. Why Innovate Physical Therapy? Physical & Other Requirements Must be able to speak, read, and write English. Must be able to independently lift up to 35 pounds. Must be able to perform repetitive sitting, bending, standing, and movement as needed. Must be flexible, calm, and professional in stressful situations. Must be able to work in a fast-paced environment with strong attention to detail. Must be able to utilize technology and resources efficiently and effectively.
