In this vital role, you will evaluate and develop personalized treatment plans to help individuals of all ages improve their daily functioning and achieve greater independence. Whether working in inpatient settings, outpatient clinics, schools, or home health environments, you will play a key role in enhancing quality of life through targeted therapy interventions. Your expertise will support patients across diverse populations, including pediatrics, individuals with disabilities, and those recovering from illness or injury. Responsibilities Conduct comprehensive assessments of patients' physical, cognitive, and emotional needs to inform tailored care plans Assist patients with Activities of Daily Living (ADLs) such as dressing, grooming, feeding, and mobility to promote independence Develop and implement effective therapy strategies for individuals with autism, disabilities, or recovering from acute care episodes Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless transitions across care settings Engage in case management activities to monitor progress, adjust treatment plans, and document patient outcomes accurately Provide therapy services in various settings including inpatient units, skilled nursing facilities, outpatient clinics, schools, and home health environments Maintain thorough documentation of patient care activities, progress notes, and treatment plans following medical terminology and anatomy knowledge standards Experience Proven experience working with diverse populations such as pediatrics (including early intervention and school-based settings), geriatrics, and individuals with disabilities Hands-on experience in acute care environments such as ICU or inpatient units is highly desirable Familiarity with post-acute care settings including skilled nursing facilities and home health services Knowledge of physiology, anatomy, medical terminology, and speech therapy principles relevant to occupational therapy practice Prior involvement in outpatient or clinic-based therapy programs demonstrating patient care excellence Experience with care planning for patients requiring discharge planning or case management support is preferred Demonstrated ability to work effectively within multidisciplinary teams to deliver comprehensive patient-centered care Join us to empower individuals through expert occupational therapy services that foster independence and improve quality of life.
