Salary:
$5,000•$7,000 Job Description Provide Occupational Therapy services for assigned inpatient, outpatient, and home care caseloads. Provide coverage for assigned wards and ensure continuity of care. Conduct initial assessments and periodic reassessments for active rehabilitation and maintenance cases. Develop individualized rehabilitation goals and treatment plans. Perform weekly reviews, treatment updates, and discharge planning. Follow up with patients under supervision. Participate in case conferences, team meetings, and family meetings. Comply with clinical guidelines and ensure proper handover arrangements during leave coverage. Collaborate with multidisciplinary teams to support patient care. Ensure the safe use of rehabilitation equipment and report defects or incidents when required. Educate patients, family members, and caregivers on rehabilitation techniques and therapeutic carry-over. Provide caregiver training when necessary. Apply evidence-based assessments and treatment interventions under supervision. Handle daily billing and organize caseload distribution. Maintain accurate and up-to-date records, including assessments, reassessments, treatment plans, memos, and incident reports in the Infocare system. Comply with Nursing Home Standards Guidelines and PDPA requirements. Participate in continuing professional development, in-service training, and community health education programmes. Assist in collecting and maintaining statistical information for regular reporting. Support patient engagement activities and observe referral trends. Requirements Diploma, Bachelor's Degree, or Master's Degree in Occupational Therapy recognized by the Singapore Allied Health Professions Council (AHPC). Registered with AHPC and possess a valid Practising Certificate. Possess a valid
BCLS + AED
certification. Minimum 1-2 years of relevant experience in a hospital or nursing home setting. Experience in community care and aged care settings will be an advantage. Postgraduate qualifications in Occupational Therapy or related fields will be advantageous. Proficient in Microsoft Office applications. Able to travel between clients' homes when required. Experience with interRAI is required.
