Job Description

Overview We are seeking a dedicated and compassionate Occupational Therapist to join our home health team. The ideal candidate will have a strong background in patient care, with expertise in various settings including home community care. As an Occupational Therapist, you will be responsible for developing and implementing effective treatment plans to enhance the physical well-being of patients, particularly in geriatrics. Duties Conduct comprehensive assessments of patients to determine their occupational therapy needs. Develop individualized care plans based on assessment findings and patient goals. Implement therapeutic interventions to improve health management for improving self care, home safety, functional cognition/mental wellness and overall function. Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes. Collaborate with multidisciplinary teams to coordinate patient care and discharge planning. Educate patients and their families about treatment options, home programs, and strategies for managing their conditions. Maintain accurate documentation of patient progress, treatment plans, and medical terminology in compliance with healthcare regulations. Participate in ongoing professional development to stay current with best practices in occupational therapy. Requirements A degree in Occupational Therapy from an accredited institution. Valid state licensure or eligibility for licensure as an Occupational Therapist. Strong knowledge of medical terminology and systems related to occupational therapy. Experience in discharge planning and working within home health care settings is preferred. Familiarity with geriatrics and the unique needs of older adults is highly desirable. Excellent communication skills, both verbal and written, for effective patient interaction and collaboration with healthcare teams. Ability to develop comprehensive care plans tailored to individual patient needs. Commitment to providing high-quality patient care in a home environment.
Pay:
$68.00 - $78.00 per hour
