Full Autonomy | Field-Based (Home + Facility) | Full-Time About the Opportunity This is not traditional home health. We are a private, boutique, concierge-style home health organization built around one core belief: exceptional patient care starts with exceptional clinician support. The Role As an Occupational Therapist (OT), you will provide skilled occupational therapy services to patients throughout Cuyahoga County in private homes and select facility settings. This is a highly autonomous, field-based role where you will: Manage your own patient caseload Build your own daily schedule Coordinate care plans with physicians, families, and interdisciplinary teams Complete documentation independently Deliver exceptional patient-centered care in the home setting Our clinicians are never left without work—caseload volume is steady, reliable, and designed for long-term success. Territory You will service Western Cuyahoga County exclusively , with territory assignments built around your starting location. Primarily including Downtown Cleveland and west-side communities such as Lakewood, Rocky River, Westlake, Bay Village, Parma, North Olmsted, and Fairview Park. Our territory model is designed to: Minimize drive time Maximize productive visits Keep clinicians close to home Create efficient, predictable daily routes No unrealistic multi-county coverage. No excessive windshield time.
Compensation:
You Control Your Income This position uses a point-based compensation model rather than hourly pay.
Pay Rate:
$60 Per Point Full-time clinicians are expected to maintain 30 points per week minimum , with unlimited upside based on productivity.
Example Earnings:
30 points/week: ~$93,600/year 35 points/week: ~$109,200/year 40 points/week: ~$124,800/year 45+ points/week: ~$140,000+ annually Most full-time OTs realistically earn $95,000-$130,000+, with top performers regularly exceeding $140,000.
