In this vital role, you will assess individual needs, develop personalized care plans, and deliver therapeutic interventions that promote independence and functional ability. Your work will empower patients of all ages— from pediatrics to geriatrics— helping them regain skills for daily living and adapt to their environments with confidence. Responsibilities Conduct comprehensive evaluations of patients' motor skills, sensory processing, and functional abilities within the home setting Develop and implement tailored treatment plans focused on improving activities of daily living (ADLs), mobility, and overall independence Assist patients with ADLs such as dressing, grooming, feeding, and toileting to maximize their autonomy Coordinate discharge planning and collaborate with interdisciplinary teams to ensure seamless transitions across care settings Work with individuals with disabilities and special needs, including those with autism or developmental delays, to foster skill development and adaptive strategies Manage case documentation accurately, including progress notes, care plans, and treatment summaries in compliance with regulatory standards Educate patients and caregivers on therapeutic exercises, safety measures, and adaptive techniques to support ongoing progress at home Experience Proven experience in home health or outpatient therapy environments, demonstrating adaptability across diverse patient populations Background working with individuals across the lifespan— from pediatric early intervention programs to geriatric geriatrics— with a focus on patient-centered care Hands-on experience in acute care settings such as ICU or inpatient units is highly valued for understanding complex medical conditions and physiology Familiarity with skilled nursing facilities or post-acute care environments enhances your ability to coordinate comprehensive discharge plans Knowledge of medical terminology, anatomy, physiology, and the use of speech therapy techniques when applicable is essential for holistic patient assessments Prior involvement in outpatient clinics or school-based programs provides insight into working within multidisciplinary teams and educational settings Strong case management skills coupled with excellent communication abilities ensure effective collaboration with healthcare providers, families, and caregivers Join us as a Home Health Occupational Therapist if you're passionate about delivering compassionate care that transforms lives.
