Full-time, PERM Key Responsibilities:

Conduct comprehensive assessments of patients' functional abilities related to daily living activities (ADLs) and instrumental activities of daily living (IADLs). Develop and implement individualized treatment plans to improve motor skills, cognitive function, and independence. Collaborate with interdisciplinary teams to coordinate patient care and ensure holistic management. Document patient progress accurately and in compliance with regulatory standards. Educate patients and families on therapeutic strategies and adaptive equipment to support ongoing recovery and independence.

Required Qualifications:

Valid Occupational Therapist license in the state of Oregon. Minimum of 6 months of experience in SNF or LTC settings. Strong clinical skills, particularly in geriatrics and long-term care. Excellent communication, assessment, and documentation skills. Ability to work effectively in a team and adapt in a fast-paced environment. Preferred Experience /

Skills:

Experience with geriatric populations in skilled nursing or long-term care facilities. Knowledge of regulatory requirements for SNF/LTC settings.
