Job Description

Occupational Therapist Elizabethtown What you'll do Conduct comprehensive in-home assessments to evaluate patients' physical, cognitive, and functional abilities and establish individualized plans of care. Develop and implement personalized treatment plans targeting activities of daily living (ADLs) and instrumental ADLs, with short- and long-term goals aligned to each patient's needs. Assess the home environment and recommend modifications, adaptive equipment, and safety improvements to promote independence and reduce fall risk. Deliver skilled therapeutic interventions focused on self-care, mobility, strength, cognition, and functional recovery — including for patients with complex neurologic diagnoses. Educate patients, caregivers, and family members on therapeutic techniques, home exercise programs, and strategies to support ongoing independence. Complete timely, accurate clinical documentation including OASIS assessments, progress notes, and discharge summaries using tablet-based platform. Monitor patient progress continuously and adjust treatment plans as needed to achieve optimal functional outcomes. Collaborate with physicians, nurses, physical therapists, speech therapists, and other interdisciplinary team members to coordinate seamless patient care. Supervise and delegate appropriately to Certified Occupational Therapy Assistants (COTAs) as applicable. Ensure all care is delivered in compliance with agency policies, state regulations, and Medicare/Medicaid standards. Qualifications Knowledge of rehabilitation techniques related to complex neurologic injury Experience with home health, medical-surgical, or rehabilitation center environment
Required Certifications and Licensures:
Master's degree in Occupational Therapy or the equivalent with license or certificate within the state providing services At least two years of experience working independently under minimal supervision Must be a licensed driver who can travel to all business locations
