Job Description

Key Qualifications:
Current and valid Occupational Therapist (OT) license 1+ year of professional experience, with proven skills in home health occupational therapy preferred Strong assessment, care planning, and documentation abilities Ability to work both independently and as part of an interdisciplinary care team Excellent communication and interpersonal skills Reliable transportation and willingness to travel to patient residences
Primary Responsibilities:
Evaluate patient needs and environments to create customized, evidence-based care plans Deliver compassionate, hands-on occupational therapy services focused on improving daily living skills and functional mobility Set measurable treatment goals and monitor progress, adjusting plans as needed Collaborate effectively with nurses, physical therapists, physicians, and family members Accurately document all services rendered according to best practices and regulatory requirements Educate patients and their caregivers on adaptive techniques and home safety strategies
