Job Description

Optimal Home Rehab LLC is looking for experienced PRN and part time occupational therapists to to provide services in West Philadelphia and parts of Delaware County. In home rehab experience is
REQUIRED.
Provides occupational therapy services on an intermittent basis to patients in their homes in accordance with physician's orders. The occupational therapist is responsible for supervising and participating in the application of a wide variety of treatments to enhance rehabilitation of patients with physical disabilities, injuries or diseases. Job Responsibilities Performs comprehensive assessment at the initial visit, re-evaluation visits, discharge and as needed. Provides occupational therapy services as prescribed by a physician in the plan of care, which can be safely provided in the home and assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional abilities tests. Develops and evaluates the patient's plan of care in partnership with the patient, the patient's representative (if any) and the patient's caregivers. Assists the physician in evaluating disability, injury or disease process of individual patients, and determining the patient's level of function. Evaluates the home environment and makes appropriate recommendations. Assists the physician in evaluating level of function, helps develop the plan of care (revising as necessary), prepares clinical and progress notes, advises and consults with the family and other agency personnel, and participates in agency sponsored in-services. Provides ongoing interdisciplinary assessment of the patient. Performs and documents basic clinical assessment every visit to include vital signs. Guides the patient in the use of therapeutic, creative and self-care activities for the purpose of improving function. Instructs the patient, caregivers and other health team personnel, when appropriate, in therapeutic procedures of occupational therapy. Maintains confidentiality of all patient, agency and employee information. Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education Registered, licensed or certified by the state of Pennsylvania as an occupational therapist after graduating from an accredited institution. Home care experience preferred. Experience A minimum of one year as an Occupational Therapist. Outpatient and or/Home Health experience is preferred.
Pay:
$65.00 - $80.00 per hour
