  • Part-time, PRN, Per diem We are seeking an energetic Occupational Therapist with leadership qualities, to join our growing team.
The Occupational Therapist will assess functional activity demands against the performance ability of the client. They will identify and address deficits to improve the client's quality of life and rebuild or maintain the client's independence.
Responsibilities:
Work cooperatively with physicians to develop an appropriate treatment and car e plan specific to the client and their physicians report Assess the home environment, especially as it related to fall risk and intervention and home safety Identify equipment needs and develop adaptive plan to improve client function and independence Report client's response to treatment or changes in condition to the Director of Nursing and physician As appropriate, coordinate interventions with other health care providers and live setting such as worksite, adult day care or residential setting while instructing appropriate personnel, family and caregivers, on intervention goals Prepare clinical and progress summaries as well as discharge planning. Submit documents to Director of Nursing Treat clients, family, and staff with courtesy and respect
Qualifications:
A current PA Occupational Therapist license A minimum of one year of recent work experience as an Occupational Therapist Ability to work independently and manage time effectively Strong interpersonal skills, organizational skills, and problem solving skills Solid computer skills; prior experience with electronic medical records (EMR) preferred
