Occupational Therapist — Founding Clinical Team 2 positions available Columbia, MD (on-site) 1099 contractor Founding cohort hire Functional independence is not a side effect of good rehabilitation — it is the point. Help build the occupational therapy program at Maryland's first non-hospital-affiliated CORF. About AthleticsMD® AthleticsMD® is a physician-founded organization establishing the first non-hospital-affiliated Comprehensive Outpatient Rehabilitation Facility (CORF) in Maryland . Operating under Maryland COMAR 10.07.18 and Medicare conditions at 42 CFR §§ 410.100-410.105, the facility delivers integrated, physician-directed, multidisciplinary rehabilitation across two patient populations: surgical patients in the four-to-eight-week preoperative window (prehabilitation), and post-acute patients transitioning from hospital or subacute rehabilitation. Medical direction is provided by a Johns Hopkins-trained cardiac anesthesiologist and critical care physician; surgical leadership comes from a University of Maryland Medical Center / R Adams Cowley Shock Trauma Center-trained general surgeon. This is a ground-floor opportunity to help build a program of statewide significance. The clinicians hired in this first cohort will shape clinical protocols, intake workflows, outcomes measurement, and the relationships with referring surgical teams across Maryland. Role summary The Occupational Therapist is one of six core clinical disciplines this facility will offer, exceeding the
COMAR 10.07.18
minimum. You will design and deliver occupational therapy plans that prepare surgical patients for functional demands of recovery and that rebuild independence for post-acute patients. Your judgment about ADL trajectory, home environment, durable medical equipment, and adaptive technique will directly shape patients' return to home, work, and community. What you'll do Perform OT evaluations at admission addressing
ADL/IADL
function, upper extremity strength and coordination, cognition, visual-perceptual function, sensory integration, and home/community environment. Build individualized treatment plans focused on functional task training, energy conservation, joint protection, adaptive equipment training, home modification recommendations, and return-to-work or return-to-role preparation. Coordinate closely with physical therapy on shared patients, particularly around transfer training, balance, fall prevention, and equipment use. Lead patient and caregiver education sessions on adaptive techniques and equipment. Participate in monthly interdisciplinary case conferences. Contribute to development of OT protocols specific to prehabilitation (preoperative ADL preparation, equipment pre-trial) and post-acute recovery (driving readiness, return-to-work staging). Maintain documentation compliant with Medicare CORF, Maryland Occupational Therapy Practice Act, and the facility's quality improvement program. Scope of practice Practice occurs within the full scope of the Maryland Occupational Therapy Practice Act and the Maryland Board of Occupational Therapy Practice regulations. All services are provided under physician-directed plans of care as required by federal CORF regulation. Required qualifications Master of Occupational Therapy (MOT) or Doctorate (OTD) from an ACOTE-accredited program. Current, unrestricted Occupational Therapist license in the State of Maryland (or eligibility for immediate licensure by endorsement). NBCOT certification (OTR/L). Current BLS certification. Demonstrated experience with adult outpatient rehabilitation, post-surgical recovery, or neurorehabilitation populations. Clean background check and current immunization records per federal CORF requirements. Preferred qualifications Experience with hand therapy, driving rehabilitation, or work conditioning/work hardening programs. Familiarity with COPM, FIM, Modified Barthel Index, or other validated functional outcome instruments. Experience supervising COTA practice in a regulated outpatient environment. Prior involvement in clinical program design or protocol development. Schedule, location, and compensation On-site at 8815 Centre Park Drive, Columbia, MD 21045. This is an in-person role; remote work is not available for clinical positions.
Standard hours:
Monday-Friday, 9:00 AM - 4:00 PM. Occasional Saturday availability by appointment only.
Compensation:
$40 per hour (1099 independent contractor), with the $5,000 Founding Partnership Bonus described below. AthleticsMD® complies with all applicable U.S. federal and Maryland state laws and regulations governing healthcare employment, contractor classification, clinical licensure, patient privacy (HIPAA), and rehabilitation facility operations. All clinical work is performed under physician-directed plans of care consistent with 42 CFR § 410.105 and
COMAR 10.07.18.
Partnership bonus Each clinician hired into this founding cohort is eligible for a $5,000 Founding Partnership Bonus payable once the program reaches full operational status (defined as full Maryland CORF licensure, Medicare certification, and sustained patient census). This bonus is in addition to all contracted hourly compensation. Long-term outlook This engagement begins as a 1099 independent contractor role. As the program matures, AthleticsMD® intends to transition committed founding clinicians toward W-2 employment status with a benefits package — including health, retirement, and CEU support — to be defined and mutually agreed upon. The current contractor structure reflects the early-stage capital posture of a brand-new licensed facility, not a long-term staffing philosophy.
