Occupational Therapist, OT (Per Diem / PRN) Golden Crest Nursing Centre 100 Smithfield Road, North Providence, RI 02904 Job Type Per Diem / PRN (As Needed) Position Summary Golden Crest Nursing Centre is seeking a compassionate and skilled Occupational Therapist to join our in-house subacute rehabilitation team on a Per Diem / PRN basis. The Occupational Therapist will provide evaluation and treatment services to residents in our skilled nursing and subacute rehabilitation facility, helping them achieve their highest level of independence and functional ability. Responsibilities Evaluate residents and establish individualized treatment plans. Provide skilled occupational therapy interventions in accordance with physician orders and regulatory standards. Assess and treat deficits related to activities of daily living (ADLs), upper extremity function, cognition, and safety awareness. Collaborate with physicians, nursing staff, therapy team members, residents, and families. Maintain timely and accurate documentation in accordance with facility, state, and federal requirements. Participate in interdisciplinary team meetings and care planning.
