DESCRIPTION
Occupational Therapist (OT)Job Description:
The Occupational Therapist plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.Responsibilitis:
Assessment and Evaluation:
Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.Individualized Care Plans:
Develop individualized treatment plans based on assessments, with clear goals for improving residents' functional mobility.Treatment:
Administer therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.Education:
Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.Documentation:
Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( InnovaHealth/Net Health/PCC).Interdisciplinary Collaboration:
Collaborate with the rehab team, including Physical Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.Qualifications:
State licensure as an Occupational Therapist. EMR and MS office applications. CPR/BLS certification. Strong clinical and interpersonal skills. Excellent written and verbal communication skills. Ability to supervise assistants, technicians/aids and student/interns. #IND1Pay:
