Cheraw Post Acute 400 Moffat Road Cheraw, SC 29520 Join Our In-House Therapy Team! Cheraw Post Acute is seeking a dedicated and compassionate Full-Time Occupational Therapist (OT) to join our exceptional in-house therapy team. PRN opportunities are also available.

Facility:

132-Bed Skilled Nursing Facility (SNF) If you are passionate about helping patients achieve their highest level of independence and quality of life, we would love to hear from you! Responsibilities Assess patients' physical abilities and limitations through comprehensive evaluations. Develop individualized treatment plans based on patient needs and goals using evidence-based practices. Implement therapeutic exercises and interventions to improve mobility, strength, and overall function. Educate patients and families regarding treatment plans, home exercise programs, and self-management strategies. Collaborate with interdisciplinary team members to ensure comprehensive patient care and effective discharge planning. Maintain accurate and timely documentation of patient progress, treatment interventions, and outcomes in compliance with regulatory standards. Stay current with advancements and best practices in occupational therapy. Qualifications Master of Occupational Therapy (MOT) or Doctor of Occupational Therapy (DOT) degree from an accredited program. Current state licensure or eligibility for licensure as an Occupational Therapist. Strong knowledge of medical terminology and occupational therapy practices. Experience in a skilled nursing facility (SNF) setting preferred but not required. Familiarity with discharge planning and transitional care processes. Experience treating geriatric populations is a plus. Excellent communication and interpersonal skills. Why Join Cheraw Post Acute?
