Job Description

Join Our Team at The Seabrook of Hilton Head The Seabrook of Hilton Head is seeking a compassionate and skilled PRN Occupational Therapist (OT) to join our rehabilitation team. This position offers flexible scheduling and the opportunity to make a meaningful impact in the lives of our residents through high-quality therapeutic care. Responsibilities Assess patients' physical and cognitive abilities to develop personalized care plans that promote independence in activities of daily living (ADLs) such as dressing, grooming, and feeding. Implement therapeutic interventions tailored to individual needs, including working with individuals with autism, disabilities, geriatrics, pediatrics, and those recovering from acute care or ICU stays. Assist patients with discharge planning by coordinating with multidisciplinary teams to ensure smooth transitions from hospital to home or other care facilities. Conduct evaluations and provide therapy in various settings such as inpatient units, skilled nursing facilities, outpatient clinics, schools, and home health environments. Collaborate with case management teams to monitor progress, adjust treatment plans as necessary, and ensure comprehensive patient care. Maintain detailed documentation of patient progress, care plans, and treatment outcomes in compliance with healthcare regulations. Support patient education initiatives related to physiology knowledge, medical terminology, anatomy understanding, and safety strategies for improved health outcomes. Requirements Valid occupational therapy license or certification in the relevant state or region. Proven experience working with diverse populations including geriatrics, individuals with autism or disabilities, and post-acute care patients. Strong knowledge of physiology, anatomy, medical terminology, and therapeutic techniques applicable across inpatient and outpatient. Experience assisting with ADLs and discharge planning within acute care or skilled nursing environments is highly preferred. Ability to work independently as needed while collaborating effectively within multidisciplinary teams. Excellent communication skills for working with patients of all ages and backgrounds. Flexibility to adapt to various clinical environments such as ICU units, outpatient clinics, early intervention programs, or school settings. Pay:
$50.00 - $55.00 per hour
