Job Description

Job Overview We are seeking a passionate and dedicated Occupational Therapist to join our dynamic healthcare team. In this vital role, you will assess, plan, and implement personalized therapy programs to improve patients' functional abilities in the home health setting. Your expertise will help individuals of all ages overcome physical, cognitive, and sensory challenges, empowering them to achieve greater independence and quality of life. This position offers an exciting opportunity to make a meaningful difference through innovative therapeutic interventions and compassionate patient care. Duties Conduct comprehensive evaluations of patients' physical, cognitive, and sensory functions using established assessment tools and clinical observations. Develop individualized care plans tailored to each patient's unique needs, goals, and medical conditions. Implement evidence-based therapeutic techniques to enhance motor skills, coordination, sensory processing, and daily living activities. Assist patients with activities of daily living (ADLs), including dressing, grooming, feeding, and mobility support. Collaborate with multidisciplinary teams—including physicians, speech therapists, physical therapists, and social workers—to coordinate holistic care strategies. Facilitate discharge planning by preparing patients for transition to home or other care settings, ensuring continuity of therapy services. Provide therapy in diverse environments such as inpatient hospitals, outpatient clinics, skilled nursing facilities, home health visits, early intervention programs, schools for children with disabilities or autism, and occupational health settings. Maintain accurate documentation of patient progress, treatment plans, and case management records in compliance with healthcare regulations. Stay current with advances in physiology, anatomy, medical terminology, and therapy techniques to deliver high-quality care. Skills Strong knowledge of physiology and anatomy relevant to occupational therapy practice. Proficiency in medical terminology and understanding of various health conditions affecting mobility and cognition. Experience assisting with ADLs and developing effective discharge plans. Skilled in working with individuals across the lifespan—pediatrics (including autism spectrum disorders), geriatrics, post-acute care patients, and those with disabilities. Familiarity with acute care environments such as ICU settings and inpatient units; ICU experience is highly valued. Background in outpatient clinics or home health services; clinic experience is preferred. Ability to create comprehensive care plans tailored to individual needs within skilled nursing facilities or outpatient settings. Excellent case management skills for coordinating multidisciplinary care teams effectively. Knowledge of speech therapy principles can be an asset when collaborating on holistic patient rehabilitation. Pay:
$55.45 - $65.55 per hour
