Position Summary The Occupational Therapist will evaluate, plan, and provide skilled therapy services to residents and patients with physical, cognitive, or functional limitations. The ideal candidate will have strong clinical skills, a patient-focused approach, and a passion for helping individuals maximize independence in daily living activities. Responsibilities Evaluate residents' physical, cognitive, and functional abilities and determine therapy needs Develop individualized treatment plans designed to improve safety, mobility, independence, and overall quality of life Implement therapeutic interventions to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs) Assess home and environmental needs and provide recommendations for adaptive equipment and modifications Educate patients, caregivers, and family members on treatment goals, safety techniques, and adaptive strategies Monitor patient progress and adjust treatment plans as clinically appropriate in collaboration with the interdisciplinary team Maintain accurate, timely, and compliant documentation according to company policies and state/federal regulations Collaborate with nursing, therapy, and medical staff to ensure quality patient care and positive outcomes Attend required meetings and participate in care planning as directed by the Director of Rehabilitation Ensure compliance with all facility policies, procedures, and professional standards Qualifications Active and valid Texas Occupational Therapy license Graduate of an accredited Occupational Therapy program Skilled nursing facility (SNF) or long-term care experience preferred Knowledge of Net Health documentation system preferred Strong communication, organization, and interpersonal skills Ability to effectively manage and treat patients with varying diagnoses and personalities Why Join Park Bend Rehab?
