We are seeking a dedicated and compassionate Occupational Therapist to join our dynamic healthcare team. In this role, you will play a vital part in helping individuals of all ages regain independence and improve their quality of life through personalized therapy interventions. Your expertise will support patients across diverse settings, including inpatient units, outpatient clinics, schools, home health, skilled nursing facilities, and post-acute care environments. This position offers an exciting opportunity to make a meaningful impact by developing tailored care plans, assisting with activities of daily living (ADLs), and facilitating discharge planning to ensure seamless transitions for our clients. Responsibilities Conduct comprehensive assessments to evaluate patients' physical, cognitive, and emotional needs using physiology knowledge and medical terminology. Develop and implement individualized treatment plans focused on improving functional abilities and promoting independence. Assist patients with activities of daily living (ADLs), including dressing, grooming, feeding, and mobility exercises. Collaborate with multidisciplinary teams to coordinate care plans, discharge strategies, and case management efforts for optimal patient outcomes. Work with individuals with disabilities, autism spectrum disorder, geriatrics, pediatrics, or those recovering from acute care or ICU stays. Provide therapy services in various settings such as inpatient units, outpatient clinics, school environments, home health visits, or skilled nursing facilities. Educate patients and their families on therapy techniques, adaptive equipment use, and safety measures to support ongoing progress post-discharge. Experience Prior experience working in acute care settings such as hospitals or ICU environments is highly desirable. Demonstrated proficiency in early intervention programs and outpatient clinics. Experience in skilled nursing facilities or post-acute care environments is preferred. Knowledge of working with individuals with autism or disabilities across different age groups is essential. Familiarity with care plans development, discharge planning processes, and case management practices. Strong understanding of physiology, anatomy knowledge, medical terminology, and patient care principles. Previous roles involving occupational health services or school-based therapy are advantageous.
