Position Summary The Occupational Therapist will provide skilled home health rehabilitation services designed to improve patients' ability to perform activities of daily living (ADLs), increase functional independence, enhance safety, and maximize overall quality of life within the home environment. Responsibilities Evaluate and treat patients with physical, cognitive, and functional limitations in the home setting Develop and implement individualized treatment plans in accordance with physician orders and established plans of care Provide skilled occupational therapy interventions to improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) Assess home environments and recommend modifications, adaptive equipment, and safety strategies as appropriate Monitor patient response to treatment and document progress toward functional goals Educate patients, family members, and caregivers regarding home safety, adaptive techniques, and therapeutic exercises Collaborate with physicians and interdisciplinary care team members to ensure coordinated patient care Maintain accurate, timely, and compliant documentation according to company policy and state/federal regulations Participate in care coordination and interdisciplinary meetings as required Qualifications Active and valid Texas Occupational Therapist license in good standing Graduate of an accredited Occupational Therapy program Home health experience preferred Skilled nursing facility (SNF) or geriatric experience preferred Knowledge of electronic documentation systems preferred Strong communication, organization, and interpersonal skills Ability to work independently and manage patient schedules effectively Career Advantages PRN flexible scheduling Supportive and collaborative clinical team Competitive compensation Opportunity to build meaningful relationships with patients Autonomy and independence in patient care Professional growth opportunities New graduates encouraged to apply Join a team that values compassionate care, clinical excellence, and professional development.
