Full-Time and/or PRN Occupational Therapist (OT) - In-House Rehab | Gilmer, TX Join Our In-House Therapy Team at Focused Care at Gilmer! We are seeking a compassionate and dedicated Occupational Therapist (OT) to join our in-house rehabilitation team at Focused Care at Gilmer. Position Overview As an Occupational Therapist (OT), you will evaluate, develop, and implement evidence-based treatment programs designed to maximize residents' independence, safety, and quality of life. You will work closely with an interdisciplinary team to deliver exceptional rehabilitative care and support positive resident outcomes. Key Responsibilities Evaluate residents to identify functional deficits, strengths, and therapy needs Develop and implement individualized treatment plans focused on ADLs, IADLs, and functional independence Monitor resident progress and modify treatment plans as appropriate Collaborate with physicians, nursing staff, therapy team members, and caregivers to optimize outcomes Recommend adaptive equipment, environmental modifications, and compensatory strategies Educate residents, families, and caregivers on therapeutic interventions and home strategies Maintain accurate and timely documentation in compliance with state, federal, and company guidelines Participate in interdisciplinary meetings, care planning conferences, and team collaboration Ensure compliance with all regulatory and facility policies Qualifications Active Texas Occupational Therapist (OT) license In good standing with all applicable licensing boards and regulatory agencies Experience in long-term care, skilled nursing, geriatric rehabilitation, or post-acute care preferred Familiarity with Net Health documentation software is a plus Strong clinical, communication, organizational, and interpersonal skills Passion for helping older adults maintain independence and quality of life Why Join FPACP?
