Occupational Therapist - PRN The Atrium Rehabilitation Center - 2.7 San Antonio, TX Job Details PRN 17 hours ago Qualifications Skilled nursing facility experience Inpatient experience Electronic health records (EHR) management Care documentation Records management Occupational Therapy Health record tracking Managing patient records Medical terminology Post-acute care experience Full Job Description Job Summary We are seeking a dedicated and versatile Occupational Therapist to provide compassionate, patient-centered care across a variety of clinical and community settings on a PRN (as-needed) basis. The ideal candidate will possess a strong foundation in occupational therapy principles, with experience working with diverse populations including individuals with disabilities, neurodevelopmental disorders, and those in post-acute or inpatient care. This role offers an opportunity to apply evidence-based practices, collaborate with interdisciplinary teams, and support clients in achieving their functional goals through tailored interventions. Responsibilities Conduct comprehensive assessments to evaluate patients' functional abilities, sensory processing, and occupational performance. Develop individualized care plans that incorporate therapeutic activities aimed at improving ADLs (Activities of Daily Living), motor skills, and cognitive functions. Assist patients with daily tasks such as dressing, grooming, and mobility to promote independence. Support discharge planning by coordinating with healthcare teams to ensure smooth transitions from inpatient or acute care settings. Work with individuals across the lifespan, including pediatrics in early intervention or school settings, as well as geriatrics and adult populations in outpatient or home health environments. Implement evidence-based interventions for clients with neurodevelopmental disorders such as autism spectrum disorders and other disabilities. Maintain accurate medical documentation and manage electronic health records (EHR) efficiently in compliance with privacy regulations. Collaborate with multidisciplinary teams including speech therapists, physical therapists, case managers, and educators to optimize patient outcomes. Provide education and support to families, caregivers, and educators on strategies for supporting individuals with disabilities or special education needs. Engage in ongoing professional development to stay current with advances in occupational therapy practices and physiology knowledge. Experience Proven experience working in inpatient settings such as hospitals or skilled nursing facilities is highly desirable. Experience providing therapy within post-acute care environments including home health or outpatient clinics. Background working with pediatric populations in early intervention programs or school-based settings is preferred. Familiarity with working with individuals who have neurodevelopmental disorders such as autism spectrum disorder. Knowledge of medical terminology, anatomy, and physiology relevant to occupational therapy practice. Demonstrated ability to develop care plans tailored to individual needs and case management skills for complex cases. Prior experience managing medical documentation accurately within electronic health records systems is essential. Exposure to acute care environments such as ICU or inpatient units will be considered an asset. Ability to work independently on a PRN basis while maintaining high standards of patient care and professionalism.
