PRN Occupational Therapist (OT) - In-House Rehab Team Avir at The Lakes in Texas City, TX is excited to offer an opportunity for a PRN Occupational Therapist to join our dedicated in-house rehabilitation team. Must have weekend availability. Position Overview As an Occupational Therapist , you will assess and treat residents to improve function, independence, and overall quality of life. This role includes developing evidence-based treatment plans, delivering skilled therapy services, and collaborating closely with the interdisciplinary team. Key Responsibilities Evaluate residents to determine functional impairments and therapy needs. Develop and implement individualized treatment plans focused on ADLs and IADLs. Track progress and modify treatment plans as needed in coordination with the IDT. Recommend adaptive equipment and home modifications to promote independence. Educate patients, caregivers, and families on therapeutic strategies. Ensure accurate, timely documentation per state, federal, and facility guidelines. Participate in team meetings and care planning activities. Adhere to all regulatory and facility policies. Qualifications Active Texas Occupational Therapist license. In good standing with all applicable licensing boards. SNF or long-term care experience preferred. Familiarity with Net Health is a plus. Strong communication and interpersonal skills.
