Schedule:
Monday-Friday, 7:45 AM-4:00 PM No weekends, no on-call, and no holiday coverage requiredKey Qualifications:
Licensed Occupational Therapist in Wisconsin Active BLS certification Bachelor's degree in Occupational Therapy (minimum required) At least 3 years' OT experience (SNF, acute care, or home health strongly preferred) Valid driver's license, reliable vehicle, and active auto insurance Strong clinical evaluation skills with a focus on ADLs and adaptive equipment Competence inEPIC EMR
(preferred, with training available)Responsibilities:
Independently manage a daily caseload averaging 6 home health adult patients across Kenosha and Racine Counties Conduct evaluations and ongoing therapy for a range of conditions (geriatrics, post-stroke, joint replacement, deconditioning, COPD, CHF) Collaborate closely with RN and PT interdisciplinary teams through morning huddles and coordinated care planning Maintain thorough and timely documentation using EMR systemsBenefits & Travel Perks:
