Job Description
Thank you for viewing our job posting! We have an immediate need (current provider is moving out of state) for a passionate and outgoing COTA or OT, to help serve children and young adults with Intellectual and Developmental Disabilities (ID/DD), in a Home-Health setting. Even if you're not able to takeover the entire caseload, but are interested in a few of them, please contact us! There is also an opportunity to grow the size of your caseload, to a level you're comfortable with. We can tailor the search for additional kiddos to fit your desired location and schedule/availability. This position is currently offering a caseload of 12 kiddos, located in the following zip codes: 85374 (2 kiddos) 85355 (2 kiddos) 85387 (2 kiddos) 85388, 85378, 85335, 85345, 85379, 85383 Compensation and Bonuses $68-75 per visit/hour (Licensed OT rates would be higher) $1500 sign on bonus (you would qualify after 60 days of service with the agency) Application requirements Arizona Fingerprint Clearance Card Must be currently licensed as a COTA or OT in the State of Arizona One year experience is preferred, but not required Imagine the perks!
