Job Description

FORM Hand Therapy specializes in the rehabilitation of Hand, Wrist, Elbow conditions. FORM Hand Therapy understands the reliance on your hands and upper extremities in daily life and the significant impact a condition or trauma can cause. Thus, we offer our patients personalized, hands-on rehabilitation of injuries or conditions involving the hand, wrist, elbow utilizing a wide range of treatment methods. We provide compassionate care that truly can only be found at FORM Hand Therapy. FORM Hand Therapy clinic is seeking a hand / occupational therapy aide to assist in providing patient care services under the direct supervision of a hand/occupational therapist. The hand therapy aide will assist in preparing treatment areas, patients, equipment, and materials, and will support the provider in patient treatment.
Responsibilities:
  • Prepares treatment areas, equipment and provides hand therapy treatment for the patient under the order, direction and immediate supervision of a licensed/certified Hand Therapist. The therapist will determine the patient related tasks that can be safely and effectively performed by the aide.
  • Responsible for maintaining a safe and clean working environment and treatment area. The Hand Therapy/Occupational Therapy Aide will also be responsible for keeping inventory of department supplies, anticipating needed supplies, completing supply requisitions and verifying receipt of supplies.
  • Attends meetings and Participates in education programs and in service meetings as required.

Minimum Education:
High School or Equivalent, some college preferred.
License/Certification :
BLS
  • Basic Life Support Course(s) in anatomy, physiology or kinesiology
Minimum Experience:
1 year experience in a rehabilitation therapy aide setting or equivalent. Familiar with physical agent modalities such as ultrasound or
