PT / PTA

Provide skilled physical therapy services in accordance with physician orders and plans of care Implement therapeutic exercise programs focused on mobility, strength, balance, and functional independence Educate patients and caregivers on fall prevention and home safety Monitor and report patient progress to the interdisciplinary team For

OT / COTA

Provide skilled occupational therapy services to improve daily living and functional independence Address ADLs, upper extremity function, cognitive skills, and home safety Train patients and caregivers on adaptive techniques and equipment Collaborate with the care team to optimize patient outcomes

All Roles:

Complete accurate and timely documentation using WellSky/Kinnser EMR enhanced with NestMed AI technology Communicate effectively with physicians, nurses, and other team members Qualifications Active Florida license (PT, PTA, OT, or COTA) Home health experience preferred (training available) Reliable transportation and valid driver's license Strong clinical and communication skills Ability to work independently and manage time effectively Apply Today Positions are filling quickly.
