Duties Assess patients' physical, occupational, or functional needs through comprehensive evaluations. Develop and implement individualized treatment plans aligned with each patient's goals and medical conditions. Deliver hands-on therapy interventions, including exercises, mobility training, and adaptive techniques. Educate patients and their families on proper techniques, safety measures, and home modifications to promote ongoing health. Monitor progress regularly, adjusting treatment plans as needed to optimize outcomes. Document all patient interactions accurately in compliance with healthcare regulations and organizational standards. Collaborate with physicians, caregivers, and other healthcare professionals to ensure cohesive patient care. Requirements Current licensure or certification as a Physical Therapist (PT), Physical Therapist Assistant (PTA), Occupational Therapist (OT), or Certified Occupational Therapy Assistant (COTA). Proven experience providing therapy services in a home health or outpatient setting preferred. Strong knowledge of therapeutic techniques, safety protocols, and documentation standards. Excellent communication skills for engaging with patients and coordinating with healthcare teams. Ability to work independently with minimal supervision while managing a flexible schedule. Compassionate attitude with a genuine desire to improve patients' well-being. Valid driver's license and reliable transportation are often necessary for home visits.
