Job Description

TriSource is seeking a Rehabilitation Specialist to to support adults with mental health diagnoses in a community-based day program setting. This position focuses on helping individuals develop independent living skills, achieve personal goals, maintain safety, and increase overall quality of life. The rehabilitation specialist will manage a caseload of up to six clients and travel between the main office and residential homes throughout the community. Must be comfortable working with an at-risk clientele (mental illness, behavioral health).
Schedule:
Monday-Friday 9:45am-2:15pm Qualifications Experience working with individuals experiencing mental illness preferred High School Diploma or GED required Valid drivers license CPR-First Aid certification Ability to pass background screening Strong communication, documentation, and interpersonal skills Ability to work at multiple community locations Ability to bend, kneel, stand, lift, climb stairs, and maintain various physical positions throughout the shift Ability to safely assist clients as needed and respond to emergency situations Responsibilities Provide direct rehabilitative services and individualized support to clients with mental illness Teach and reinforce daily living skills, including cooking, cleaning, budgeting, hygiene, and community integration Assist clients with grocery shopping, appointments, and community activities Monitor and document client progress toward individualized goals Administer or monitor medications in accordance with program policies Identify and respond appropriately to crisis situations while maintaining a calm and professional demeanor Complete required documentation, including daily notes, progress reports, incident reports, and assessments Collaborate with treatment teams, supervisors, and community providers Support clients in developing independence and self-sufficiency Apply today! #IND1
