Human Services Social Work Full Job Description Job Summary:
The PSR role provides rehabilitative services that aim to enhance the social, emotional, and functional skills of children/youth, fostering independence and promoting integration within family and community settings. Responsibilities Engage children/youth in activities that develop social, emotional, and daily living skills to improve functioning within the community. Implement structured interventions that support independence, enhance interpersonal skills, and promote positive behaviors. Educate and support families in reinforcing these skills at home and in the community. Facilitate the development and monitoring of individualized service plans with clear, achievable goals. Attend monthly waiver service provider meetings to ensure consistency in service delivery and stay informed on updated practices and protocols. Document services provided, including progress notes, service plans, expense logs, and other required records, submitted within prescribed timeframes. Report any incidents or concerns immediately to the supervisor to maintain client safety and service transparency. Coordinate with other service providers and the Care Manager to discuss progress, address challenges, and ensure continuity of care. Assess the suitability of community or home settings for providing services, tailoring interventions to meet the child/youth's needs.
Qualifications:
At a minimum, staff must be 18 years old and have a high school diploma, high school equivalency preferred, or a State Education Commencement Credential with a minimum of two years' experience in children's mental health, addiction, foster care and/or a related human services field; or A bachelor's degree in social work, psychology, or in related human services; or A master's degree in social work, psychology, or in related human services Experience in rehabilitative or human services, particularly with children/youth.
